NEW STUDENT APPLICATION CHECKLIST
To initiate the Academy admissions process, collectively submit the following items to:

$250 Registration Fee
Completed FACTS Tuition Management Agreement (click the Tuition Payment Info link on the left)
Copy of Birth Certificate
Immunization Records or Medical/Religious Exemption
Copy of student's most recent report card and standardized test results
Pastor Recommendation Form (click here)
Teacher Recommendation Form (click here)
PARENT INTERVIEWS:
Upon receipt of the Application materials, a parent interview will be scheduled. Both parents and the student (grades 6-8 only) are required to attend. This is your opportunity to meet our Headmaster or Assistant Headmaster and to share your personal testimony.
TESTING:
All incoming students are academically screened. Test results provide a foundational academic benchmark for appropriate class placement. Screenings administered are the Gessell Development Screening (K) or the Kauffman Test of Educational Achievement (1st-8th Grade).
Once these steps have been completed, the Academy requires the following items be on file:
Dental Card
Physical (K-8th) and/or Sports Physical (5th-8th), for those participating in sports
